What brain autopsies brains just revealed about estrogen and Alzheimer’s |
Recent research traced post-menopausal memory loss partly to falling estrogen production within brain tissue itself. Now, a large study goes further: women who used estrogen-only hormone therapy had a 35% lower risk of showing amyloid plaques, tau tangles, and neuritic plaques at autopsy, all hallmarks of Alzheimer’s.
Blood and spinal fluid biomarkers collected while participants were still alive showed lower amyloid accumulation in estrogen-only users. They also had fewer dementia diagnoses and better scores on memory tests.
“Our findings are most directly relevant to women who have had a hysterectomy by age 60 — roughly one in three women,” says study author Jennifer Bruno, PhD, of Stanford University. Women with an intact uterus take combined estrogen-plus-progestin therapy, and that combination wasn't examined here.
Not all estrogen is alike, either. Conjugated estrogen was linked to lower Alzheimer's pathology, while estradiol was not. The benefit appeared strongest in women without the APOE e4 gene variant that elevates Alzheimer’s risk. A recent Lancet meta-analysis of hormonal therapy and dementia found similarly uneven results, suggesting timing, type, and individual biology all pull in different directions.
