Trump Administration Announces Nearly $23 Million to Modernize Rural Health Centers, Expand Mobile Care, and Make It Easier to Manage Chronic Disease in Delaware

This federal investment will support new mobile health units, remote patient monitoring, cybersecurity upgrades, and expanded care teams serving rural Delaware and bringing better access and the latest technology to residents

The Trump Administration announced today that nearly $23 million in federal Rural Health Transformation Program (RHTP) funding is being delivered to strengthen healthcare delivery and expand access for rural Delawareans. The funding will support Delaware's three Federally Qualified Health Centers (FQHCs) as they modernize technology, strengthen chronic disease management, and expand services for the communities they serve. The awards will support electronic medical record upgrades, remote patient monitoring, cybersecurity, expanded care teams, and care coordination, along with two new mobile health units and a passenger van to expand access to care for rural and agricultural communities.

Kennedy, Jr. “We are putting resources directly into rural communities to strengthen the health workforce, modernize infrastructure, expand access, and bring care closer to home. We are giving states the flexibility to build solutions around the people they serve, while demanding accountability for results. That is how we strengthen rural health, improve the lives of patients and families, and Make America Healthy Again.”

“President Trump made a historic commitment to rural America, and these investments are turning that commitment into better care for patients in communities across Delaware,” said Centers for Medicare & Medicaid Services Administrator Dr. Mehmet Oz. “This is what rural health transformation should look like: federal investment that is putting patients first, driving lasting improvements and expanding healthcare for the hardworking Americans who call these communities home.”

Today’s investment was made possible through the Working Families Tax Cuts Act — passed by Republicans in Congress and unanimously opposed by Democrats — delivering a historic $50 billion investment to help states transform the way healthcare is delivered in America.

Delaware's broader RHTP strategy is designed to expand access, strengthen the rural healthcare workforce, and support sustainable care across the state. Today’s announcement is one part of the larger overall funding amount awarded to Delaware for fiscal year 2026.

Key initiatives the overall RHTP funding will focus on include:

• Expanding access to care closer to home: Supporting rural community health hubs, school-based health centers, Federally Qualified Health Centers, libraries and other familiar community locations to connect residents more easily with primary, behavioral, preventive, and specialty care.
  
  
• Strengthening the rural healthcare workforce: Building Delaware’s “Train Here, Stay Here” pipeline through medical education, rural clinical training, residency recruitment, financial support for healthcare trainees, and expanded training opportunities for nurses, community health workers, paramedics, and other clinical support professionals.
  
  
• Improving prevention and chronic disease management: Expanding diabetes care, remote patient monitoring, preventive services, and other community-based approaches to help rural residents manage chronic conditions.
  
  
• Modernizing rural healthcare delivery: Helping rural providers and FQHCs adopt value-based care models, strengthen care coordination and data exchange, expand telehealth and remote monitoring, and build the health information technology infrastructure needed to support more connected and sustainable care.

The Trump Administration is delivering on its commitment to strengthen rural America by investing in solutions that modernize, transform and expand healthcare access for rural communities nationwide.  Led by CMS, the RHTP is moving beyond the failed one-size-fits-all approach and giving rural communities the tools to build stronger, more innovative, and more sustainable health systems that deliver better care and better outcomes for the Americans they serve.
